Home2024-25 Acknowledgement

One parent/guardian from each family is required to complete the following form prior to the start of tryouts (for players already signed for the upcoming season) or upon signing an offer to play for Lakeside for the upcoming season.

As a player/family committing to Lakeside Volleyball Club for the 2024-25 Indoor Volleyball Season as a member of the Ontario Volleyball Association, we (including player, parents and/or guardians) accept and confirm the following:

Team Selection as it relates to Lakeside’s approach to building rosters.

Training and expectations as they relate to attendance and our policies regarding missed sessions.

Training (Spectators) – training is closed to spectators except on designated days/nights outlined by the coach of each team (coach’s discretion).

Fee Schedule – $500 deposit due at signing, $500-$1000 due October 1, 2024, and the balance to be paid in full on or before November 30, 2024.

Playing Time – playing time is earned and not always given out equally.

Player Re-Alignment Policy – players may be moved between rosters at any age group prior to December 1, 2024 if Club Executives deems a roster re-assignment would be beneficial for the well-being and success of the player(s), and is in the best interest of the teams involved.

Communication – you must wait 24-hours after a tournament before approaching a coach with an issue or a request to meet.

We understand the expectations of the player and family during the course of the season and will adhere to all policies set forth by the Club from the day of signing until the end of the season (May 31, 2025).

By signing below, we, the player and parent(s)/guardian(s), agree to all terms and conditions, and will abide by them for the duration of the indoor volleyball season.
